The 15th International Conference for Integrated Care "Complex Needs, Integrated Solutions: Engaging, empowering and enabling people for active and healthy living" will take place at the Sheraton Grand Hotel, Edinburgh, Scotland, 25-27 March 2015.  

The conference is organised by the International Foundation of Integrated Care, in partnership with NHS24  and the Joint Improvement Team of the Scottish Government.

 

Focus of conference

The conference will focus on research, policy and practice in equal measures and will feature a wide range of keynote presentations and paper presentations from the academic, public, private and third sectors. The main themes of the conference are:

• Engagement and co-production with individuals, carers, families, communities, care workers and decision makers. 
• Tackling multimorbidities, health inequalities and complexity with Integrated Care.
• Transforming care systems: evaluation and continuous improvement
• Digital health and innovations
