I, my Lawyer and my Estate (if I do not survive this operation) inform the surgical and care team that you are Put on Notice that before applying any anasethsia, making the first cut, or stitching me up, you will follow the protocols and guidelines relevant to your particular area of clinical and surgery practice to their full intention and purpose. 

Any blood products you transfuse must be done to the WHO Blood Safety guidelines.

If and patient harm occurs, it is my intention to sue for the sum of $1million per limb lost. 

I hope you will now do your best after careful due Diligence and following all duty of care procedures.
